          Hypnotherapy Disclosure Statement
             National Guild of Hypnotists

Contact Information: My name is Raymond Zukowski. I am doing business as a Hypnotherapist at
Body Mind Mentoring, and can be contacted at PO Box 8624, Tacoma, WA 98419 or by telephone at
253-223-9332. You may also contact me by e-mail at ray@bodymindmentoring.com.

Hypnosis Education and Training: In 2002 I was trained in Basic, Intermediate and Advanced
Hypnotherapy through Tacoma Community College by Roy Hunter. I also studied Medical and Dental
Hypnosis through Everett Community College, Michael Gershmen. I am now an active Certified
Hypnotherapist with the National Guild of Hypnotists, and I do annual continuing education to
maintain my training at a high level. The National Guild of Hypnotists is the oldest and largest
hypnotism organization in the world, and its certification is the most widely recognized credential for
the professional practice of the hypnotic arts. As a practicing Hypnotherapist, I am registered with the
Washington State Department of Health (Registration Number HP10001194).

As a Hypnotherapist, I am neither a physician nor a licensed health care provider, and may not
provide a medical diagnosis, prescribe medications, or recommend discontinuance of medically
prescribed treatments. Counselors practicing counseling (including hypnotherapy) for a fee must be
registered with the Department of Health for the protection of the public health and safety.
Registration of an individual with the Department does not include a recognition of any practice
standards, nor necessarily implies the effectiveness of any treatment. You have the right to choose a
Hypnotherapist who best suits your needs and purposes; additional services other than my own may
be available to you in the community, and you may locate such providers in the telephone book.. You
have the right to refuse hypnotism services at any time. As my client, you have a right to be free of
physical, verbal or sexual abuse. You also have a right to know the expected duration of sessions,
and may assert any right without retaliation.

Redress: As a certified member of the National Guild of Hypnotists, I practice in accordance with its
Code of Ethics and Standards of Conduct. If you have a complaint about my services or behavior that
I cannot resolve for you personally, you may contact the National Guild of Hypnotists at PO Box 308,
Merrimack, NH 03054-0308, telephone 603-429-9438, to seek redress. You may also file a formal
complaint with the Washington State Department of Health in accordance with the information
provided in "Purpose" below.

Fees: I offer a free, one-time, consultation service for each new hypnotherapy client. The normal
charge for my hypnosis-related services is $85.00 per hour, pro-rated for portions of an hour in
quarter-hour increments. I do not accept credit cards, and prefer payment as services are rendered.
Insurance: I suggest you think of my services as something that you will pay for personally. That will
both protect your privacy and help you value the work you are doing more. At this time, insurance
companies generally do not like to cover hypnotic services, and I advise you not to expect them to do
so.

Confidentiality: I will not release any personal information about the services you receive to anyone
without written authorization from you, except as provided for by law. You have the right for access to
my written records about you.

My Approach To The Use of Hypnotherapy: Hypnotherapy is one tool that can help to restore balance
and wellness in your life in several ways. Hypnotherapy has been shown to be helpful when used
under medical referral as an adjunct to medical treatments that address certain physical or
emotional conditions. In addition, hypnosis has been shown to be effective in helping clients
enhance their personal skills and abilities, regain self-confidence, explore their spirituality, determine
and set realistic goals, release apprehensions and concerns, manage situational stress in a
positive manner, regain self control, and promote increased wellness in their lives.
Purpose: The purpose of this Disclosure Statement is to comply with the requirements of the
Washington Administrative Code (WAC), Section 246-810-031 and the Counselor Credentialing Act,
Revised Code of Washington (RCW), Chapter 18.19. The purpose of the law regulating counselors
(including Hypnotherapists) is:

1. To provide protection for public health and safety; and

2. To empower the citizens of the state of Washington by providing a complaint process against
those counselors who would commit acts of unprofessional conduct. Acts of unprofessional conduct
include:
  a. The commission of any act involving moral turpitude, dishonesty, or corruption relating to the
practice of the person's profession, whether the act constitutes a crime or not. If the act constitutes a
crime, conviction in a criminal proceeding is not a condition precedent to disciplinary action. Upon
such a conviction, however, the judgment and sentence is conclusive evidence at the ensuing
disciplinary hearing of the guilt of the license holder or applicant of the crime described in the
indictment or information, and of the person's violation of the statute on which it is based. For the
purposes of this section, conviction includes all instances in which a plea of guilty or nolo contendere
is the basis for the conviction and all proceedings in which the sentence has been deferred or
suspended. Nothing in this section abrogates rights guaranteed under chapter 9.96A RCW;

  b. Misrepresentation or concealment of a material fact in obtaining a license or in reinstatement
thereof;

  c. All advertising which is false, fraudulent, or misleading;

  d. Incompetence, negligence, or malpractice which results in injury to a patient or which creates an
unreasonable risk that a patient may be harmed.  The use of a nontraditional treatment by itself shall
not constitute unprofessional conduct, provided that it does not result in injury to a patient or create
an unreasonable risk that a patient may be harmed;

  e. Suspension, revocation, or restriction of the individual's license to practice any health care
profession by competent authority in any state, federal, or foreign jurisdiction, a certified copy of the
order, stipulation, or agreement being conclusive evidence of the revocation, suspension, or
restriction;

  f. The possession, use, prescription for use, or distribution of controlled substances or legend
drugs in any way other than for legitimate or therapeutic purposes, diversion of controlled
substances or legend drugs, the violation of any drug law, or prescribing controlled substances for
oneself;

  g. Violation of any state or federal statute or administrative rule regulating the profession in
question, including any statute or rule defining or establishing standards of patient care or
professional conduct or practice;

  h. Failure to cooperate with the disciplining authority by:

       (1) Not furnishing any papers or documents;

       (2) Not furnishing in writing a full and complete explanation covering the matter contained in the
complaint filed with the disciplining authority;

       (3) Not responding to subpoenas issued by the disciplining authority, whether or not the
recipient of the subpoena is the accused in the proceeding; or

       (4) Not providing reasonable and timely access for authorized representatives of the disciplining
authority seeking to perform practice reviews at facilities utilized by the license holder;

  i. Failure to comply with an order issued by the disciplining authority or a stipulation for informal
disposition entered into with the disciplining authority;

  j. Aiding or abetting an unlicensed person to practice when a license is required;

  k. Violations of rules established by any health agency;

  l. Practice beyond the scope of practice as defined by law or rule;

  m. Misrepresentation or fraud in any aspect of the conduct of the business or profession;

  n. Failure to adequately supervise auxiliary staff to the extent that the consumer's health or safety is
at risk;

  o. Engaging in a profession involving contact with the public while suffering from a contagious or
infectious disease involving serious risk to public health;

  p. Promotion for personal gain of any unnecessary or inefficacious drug, device, treatment,
procedure, or service;

  q. Conviction of any gross misdemeanor or felony relating to the practice of the person's
profession. For the purposes of this subsection, conviction includes all instances in which a plea of
guilty or nolo contendere is the basis for conviction and all proceedings in which the sentence has
been deferred or suspended. Nothing in this section abrogates rights guaranteed under chapter
9.96A RCW;

  r. The procuring, or aiding or abetting in procuring, a criminal abortion;

  s. The offering, undertaking, or agreeing to cure or treat disease by a secret method, procedure,
treatment, or medicine, or the treating, operating, or prescribing for any health condition by a method,
means, or procedure which the licensee refuses to divulge upon demand of the disciplining authority;

  t. The willful betrayal of a practitioner-patient privilege as recognized by law;

  u. Violation of chapter 19.68 RCW;

  v. Interference with an investigation or disciplinary proceeding by willful misrepresentation of facts
before the disciplining authority or its authorized representative, or by the use of threats or
harassment against any patient or witness to prevent them from providing evidence in a disciplinary
proceeding or any other legal action, or by the use of financial inducements to any patient or witness
to prevent or attempt to prevent him or her from providing evidence in a disciplinary proceeding;

  w. Current misuse of:

       (1) Alcohol;

       (2) Controlled substances; or

       (3) Legend drugs;

  x. Abuse of a client or patient or sexual contact with a client or patient;

  y. Acceptance of more than a nominal gratuity, hospitality, or subsidy offered by a representative or
vendor of medical or health-related products or services intended for patients, in contemplation of a
sale or for use in research publishable in professional journals, where a conflict of interest is
presented, as defined by rules of the disciplining authority, in consultation with the department,
based on recognized professional ethical standards.

If you are the victim of unprofessional conduct as defined above, you may inform the Washington
State Department of Health, Health Professions Quality Assurance Division, P.O. Box 47865, Olympia
WA 98504-7865 within twenty days after determination is made. You should include the following
written information:
1. The name, address, and telephone number of the person making the report.
2. The name, address and telephone number of the counselor(s) being reported.

3. The case number of any client or patient whose treatment is a subject of the report.
4. A brief description or summary of the facts which gave rise to the issuance of the report, including
dates of occurrences.

5. If court action is involved, the name of the court in which the action is filed along with the date of
filing and docket number.

6. Any further information which would aid in the evaluation of the report.
